Welcome to East Riding Youth Dance!
This is our online membership form and must be completed by a parent/carer before their first session. Please take a few moments to complete it so your child can take part in our sessions.
Please note that a new membership form is required from all parents/carers at the start of the academic year.
The information you provide helps us:
- Keep your child safe during sessions
- Contact you quickly in case of an emergency
- Share updates about upcoming dance events and opportunities
